The Main Directorate of the State Tax Service in Donetsk region has clarified how an individual can submit an application to be included in the volunteer registry.
An application for inclusion in the Volunteer Registry can be submitted to the regulatory authority in either paper or electronic form.
In paper form, the application can be submitted personally, by mail, or through an authorized representative:
- by the individual donor personally - to any regulatory authority, presenting a passport or an identity document;
- by a representative - to the regulatory authority at the main place of registration (the residence of the individual donor), provided the representative presents an identity document, and the document proving the identity of the donor or a notarized copy, along with a notarized power of attorney for the relevant actions (which will be returned after presentation);
- by mail - to the regulatory authority at the residence of the individual donor, where the authenticity of the donor's signature on the application must be notarized.
In electronic form, application No. 1-РО (form identifier F1308303) can be submitted through the electronic cabinet or via the "My Tax" app in the "Services" section.
Application No. 1-РО must be signed electronically, based on a qualified electronic signature certificate issued to the individual donor by any qualified provider of electronic trust services.
To fill out and submit the application after identification in the private section of the Electronic Cabinet in the "For Citizens" mode, find the sub-mode "Application for Volunteers" or in the menu "Additional/Reporting Introduction/Create," select the form with code F1308303, sign, and send.
The date of inclusion of the individual donor in the Volunteer Registry is the date of the corresponding entry in the Volunteer Registry.
